Illinois Individual Estimated Income Tax Payment Form
Form IL-1040-ES is a tax document for Illinois state residents whose tax liability is more than $500 for a fiscal year. The form allows you to make an estimate on how much you owe to the state. It is recommended that you pay the full amount or else you will accrue penalties and interest.
Illinois Amended Individual Income Tax Return
Illinois Form IL-1040-X is a tax return document for taxpayers who need to amend their previous return. Also known as the amended individual tax return form, it is used to correct any errors in income, reimbursements or tax credits on a number of other Illinois IL-1040 tax documents. When filling out your IL-1040-X form, you will be given the chance to explain why you are making an amendment in your tax return.
Illinois Statement of Person Claiming Refund Due to Deceased Taxpayer Form
Form IL-1310 may be used by a spouse or personal representative to claim a refund on behalf of a deceased taxpayer. To be eligible, the spouse or representative will need to provide proof of the taxpayer’s death or a copy of the telegram or paper from the Department of Defense if the taxpayer was in active service at the time of death.

Illinois Special Depreciation Form
Form IL-4562 may be used to modify the effects of the 30 percent or 50 percent “bonus depreciation” that the IRC is allowing. In case you have filed more than one federal Form 4562, make sure to add all amounts from all federal Forms 4562, and state all amounts on Illinois Form IL-4562.
Illinois Gains from Sales of Employer's Securities of a Qualified Employee Benefit Plan
Form IL-4644 may be used by taxpayers who need to report any gains from the exchange of securities or from the sale of an employer from which you received a profit-sharing, qualified pension, or stock bonus plan. Illinois Form IL-4644 must be attached as a copy of the filed federal Schedule D to the IL-1040 or IL-1041 forms.
Illinois Substitute for Unobtainable W-2
Form IL-4852 should be filed only after you have given enough time to your employer to submit your Form W-2 and after you’ve tried multiple times to get it. A separate Form IL-4852 must be filed for each Form W-2 that was not received. When sending Illinois Form IL-4852, you will need to attach proof of withholding and wages.
Illinois Automatic Extension for Payment
Form IL-505-I must be filed only if a taxpayer did not file their tax return by the due date, if they owe a tentative tax, or if they are submitting their payment by money order or check. If an unpaid liability is found when the taxpayer is filing their tax return, the taxpayer may owe additional penalties and interest charges.
Illinois Income Tax Subtractions and Credits
Schedule 1299-C may be filed only if you are completing Form IL-1040. When filing Illinois Schedule 1299-C, you must always conserve a copy of this schedule to complete next year‘s Schedule 1299-C.

Schedule 1299-S includes a list of all enterprise zones. Enterprise zones are economically distressed areas which qualify for a special tax credit.
Illinois Recapture of Investment Tax Credits Form
Schedule 4255 may be completed if you claimed an investment credit against your earnings or replacement tax liability in a preceding year. You must recapture any such investment credit the same year in which the property became disqualified. Illinois Schedule 4255 must be attached to IL-1040, IL-1065, IL-1120, IL-1041, IL-1120-ST, IL-990-T, or IL-1023-C.
Illinois Tax Credits Form
Schedule ICR may be used by taxpayers to find out the total amount of their K-12 education expenses, property tax, and their earned income tax credits that may be claimed on Illinois Form IL-1040. If the taxpayer files the amount on Form IL-1040, Line 17 or Line 28, the taxpayer will have to attach Illinois Schedule ICR as well as other required documentation that is listed in the step-by-step instructions on the Schedule ICR form.
Illinois Schedule NR 2016
Nonresident and Part-year resident Computation of Illinois Tax
Schedule NR may be filed by part-year residents or nonresidents who want to determine the income tax that is taxed by Illinois during the tax year and to determine their Illinois tax income. Illinois Schedule NR must be filed and mailed together with Form IL-1040.
Illinois Employees Statement of Nonresidence
Form IL-W-5-NR has nothing to do with IL-1040. It actually has nothing to do with you Illinois tax return. This form simply tells your employer not to withhold any Illinois income tax from your compensation. This is required if you are a non-resident of Illinois who works in Illinois and are a resident of Iowa, Kentucky, Michigan, or Wisconsin. Illinois has certain reciprocal agreements with these states that declare that you do not need to pay taxes to the state you work in but rather to the state you are a resident of. Make sure to file Form IL-W-5-NR as your home state will not award you a tax credit if you pay taxes to Illinois for income earned in it. This means this income will avoid dual taxation unless you file Form IL-W-5-NR.
